Application for Absentee Ballot to Vote by Mail

To request an absentee ballot today:

  1. Fill out the following absentee ballot request form;
  2. Press the "Open Printer Friendly PDF" button and verify you've entered your correct information (if you made a mistake, you can edit your information directly on the PDF);
  3. Print the PDF, sign, put the completed form in an envelope with correct postage and mail it to your county board of elections (follow this link for a county board of elections directory)

It is recommended you check your registration status. You can do so (once your form is completed and submitted) by contacting your local board of elections. If your information has changed since the last time you voted and you have not yet updated your voter registration, follow this link to update your address online or if your name has changed, follow this link to complete a new voter registration.

Check only one. If the Ohio driver's license number option is selected, please provide your Ohio Driver's license number. If the last four digits of your Social Security number option is selected, please provide the last four digits of your Social Security Number. If you choose to send in a copy of your identification, be sure to send it in the envelope with your absentee ballot request.
