Form 203: Search of files with each listing and copies of each record (UCC11)
DO NOT PUT THE FOLLOWING ON YOUR FORMS:
On September 1, 2008, House Bill 46 will go into effect, giving our office the authority to reject any document containing a social security number or federal tax identification number. Please assist our office by removing all personal information from documents prior to filing them with our office.
Please note the recent revisions to the UCC filing forms. The UCC filing forms have been changed to remove fields that previously requested sensitive personal information.
