Campaign Finance Filing Forms
Ohio Campaign Finance forms are available in interactive PDF and Excel formats. Forms can be completely filled out directly from your keyboard.
All forms in PDF format require the Adobe Acrobat reader for use. The Fillable forms can be filled out on screen and printed and will assist with any necessary calculations. The Printable forms should be used to print blank and be filled in manually.
The interactive Excel forms require Excel 97 or a later version to function properly. These forms can be filled out on the screen and printed, or printed out blank and filled in manually. To save a completed form or download a blank form, utilize the "Save As" function on your computer, rename the file and save it to the desired drive or disk. Please note that only the most frequently used forms are available in the Excel format.
